The effect is as straightforward as they come:
A spectator is shown a diary that has a playing card written against every day of the year. They are asked to look up their birthday to find out what their 'birthday card' is. They can check to see that, a different date would definitely have resulted in a different card and yet, when you remove their birthday card from the deck it is the only red card in a blue deck AND it has 'Happy Birthday' written on its back!
